I am a dedicated and caring Occupational therapist who can collaborate with other health care team professionals, such as nurses, physiotherapists, speech-language therapists, and physicians, to develop an interdisciplinary treatment plan. My understanding of the patient's needs, abilities, and the environment in which he or she needs to function allows me to assist with the successful transition to the home, community, or next level of care. Although the stay in an acute care setting is brief, I start the rehabilitation process by recommending home modifications and medical equipment as part of an effective discharge plan to support functional performance. I am an excellent communicator and organiser.
