As an occupational therapist I will identify and assess your needs and agree realistic goals to help you improve your ability to do the things that are important that you want or need to do in everyday life. What I do. As an experienced community occupational therapist I work with a variety of patients in their own home to improve their quality of life. I assess patient needs and make recommendations to adapt the home environment with adaptive equipment. What my services include. • Personalised home exercise and education programme for people diagnosed with COPD to improve breathing control, increase confidence with activities of daily living. Provide equipment and assess for adaptations. Refer to other services such as dietetics or mental health services. • Offering strategies to long covid patients who are struggling with everyday life, such as fatigue management, energy conservation and meditation. • Personalised exercise programme for people diagnosed with stroke, improving movement and independent living. Referrals to other stroke services such as stroke association, speech and language, physiotherapy. I currently work in the later life and memory service completing memory assessments to support diagnosis. I work in the community and complete unscheduled and scheduled visits. I complete MDT’s with the consultant and share/deliver diagnosis to the patients. I consider client centred care with the patient and their families signposting to other services.
