As a Case Manager, I plan and manage cohesive rehabilitation and care packages to help clients make the most of their life following injury or illness. I am dedicated to facilitating clients to lead independent and satisfying lives. I quickly build rapport and ensure all involved are engaged with the rehabilitation and goal planning process.
