As clinical Team Leader I supervision, mentor and support a team of Case Managers to ensure the delivery of a high quality, creative, collaborative, client centered Case Management service. I am a BABICM advanced registered Case Manager (Occupational Therapist) able to work holistically with clients (paediatric and adults) with complex physical, developmental, psychological and medical needs. Using a client centred, goal orientated approach, I am able to develop collaborative and co-operative working relationships with clients and their support network including family, carers and other professionals. I co-ordinate acute and long- term rehabilitation following life changing injury or trauma to facilitate optimal recovery. This includes liaising and co-ordination with NHS, social care, education and independent professionals to ensure the best outcome for my clients. My experience includes managing complex packages of care, risk assessment and management and supervision of care staff and support workers. I also identify their training needs (delivering this myself, or accessing it through other providers), support staff to monitor the effectiveness of their services, and help them manage their administrative duties. One of my specialist areas of interest is supporting children (and their families) with highly complex needs (spinal cord injury, acquired brain injury and physical disability) to enjoy their childhood by attending school, developing friendship and access play. This work involves multiple agencies working collaboratively and accessing provision via Educational Healthcare Plans (EHCP) and Continuing Healthcare.
