Occupational Therapist, enthusiastic about working with individuals with complex needs independently and as part of a team. Experience in Community Occupational Therapy working with patients with a range of complex conditions including progressive neuro, cardiac and respiratory conditions, dementia, cancer, fibromyalgia, chronic fatigue syndrome, arthritis, elderly frail and patients recovering from an acute episode of their illness. Work with patients and their carers to create patient-centred treatment plans to enable them to achieve their goals. Rehabilitation includes developing strategies to enable safe transfers to maintain normal movement, prescribing equipment to assist where necessary; 24 hour posture management; manual handling requiring creative thinking and problem solving where standard methods for completing transfers and supporting bed mobility are not appropriate due to patients fluctuating condition, physical or psychosocial factors or bariatric patients; fatigue, pain and anxiety management enabling patients to improve their function, wellbeing and quality of life; rehab for hand function and hand hygiene has improved function and prevented deteriorating skin integrity and progression of contractures. Partnership working and liaison with the wider MDT across primary and secondary care. Previous experience in community older persons mental health; inpatient older persons mental health working on organic and functional wards; in-reach to an acute physical hospital and short-term physical rehab following hospital discharge.
