My daily duties are to work as part of the North Villages Neighbourhood Team and this compromises of Physio and occupational therapist and also supporting the district nurses. Due to the current high level of COVID-19 all community therapy services have been suspended so as a NT we are supporting other teams within the local hospitals and the community. I get assigned patients and visit them in their homes and this can be for: *Introducing the patients to a specific exercise programme to suit their rehab needs post surgery *Returning on a weekly bases to assess the patients progress and increase exercise program to allow patients to achieve their goals and become independent again *Ordering/collecting and delivering patients equipment they need *Fitting patients equipment in there home *Bathing equipment(shower boards,swivel bather) *Adjusting heights on walking sticks,crutches, walking frames,perching stools and kitchen trolley for the patients *Assess the patient is using their equipment correctly and advise if not demonstrate for them *Assessing bed transfers *Completing outdoor mobility with patients *Promote patients well being and independence *Liaise with the patients families/carers to make sure that the patient is getting the correct care that they require *Record all my daily patient notes on SOAP notes on Systm 1 *Support PT & OT when requiring a double up with a patient *Support and assist PT & OT when doing a larger assessment on a patient *Support and assist others using moving and handling technique when using equipment *Make sure that all equipment is in good working order and report to manager if not *When using any assessment equipment must adhere to the infection control policy
