I have carried out several rotational posts over the years. I have completed rotations in hand therapy, stroke, acute inpatient medicine, integrated discharge team and healthcare of the elderly. Gaining transferable skills which were greatly applied within the integrated discharge team where I worked to improve better patient flow. Currently working in health care of the elderly, a fast paced acute setting. Mainly focused on assessment and discharge planning. Excellent skills in rehabilitation plans and goal setting. Very experienced in working in a fast paced unit and managing heavy case loads. HCPC registered
