Occupational Therapist working in Adult care services, with older people living with physical disabilities, cognitive impairments and complex social needs, subsequently day to day tasks are difficult. I seek to enable people to achieve their best level of function and independence, in their homes and communities. Where necessary, I work closely with care agencies, GP’s, NHS Therapy & nursing teams, housing associations, families, voluntary services and hospital teams to ensure people are supported to achieve their outcomes. I will liaise with other professions to ensure unpaid carers are assessed for their own needs in order to prevent carer breakdown. Interventions include •moving and handling assessments •equipment provision, •teaching new techniques, •providing advice, •raising referrals for more specific care such as •physiotherapy, •bowel & bladder, •wheelchairs, for •minor & major adaptations •Safeguarding’s and •continuing health care
