I've been working in Adult Social Care since 2012, originally as a carer before joining Adult Social Services for the local authority. I joined Norfolk County Council where I worked with the 'front door service' for Adult Care - Social Care Centre of Expertise (SCCE) as an Assistant Practitioner. From this, I moved into a role with Norfolk Children's Services as a Family Practitioner with Norfolk Early Help and Family Focus. I returned to Adult Social Services in a specialised role, conducting Care Act Assessments to support adults being discharged from Community Hospitals in Norfolk. I progressed within the service to a Trusted Assessor, I assessed patients at Norfolk and Norwich Hospital University Hospital (NNUH) that are either returning or are new admissions to a 24-hour care setting on behalf of the provider/home manager. Following the pandemic, I made a decision to move from Norfolk to Sheffield. I worked with Short Term Intervention Team (STIT) in the screening and allocation team. I screen referrals from the acute hospitals for all Sheffield patients and liase with the hospital teams to advise when short term care can start for patients to support with discharge flow. I was successfully appointed a new role with Sheffield City Council with Equipment and Adaptations Team as an apprentice Occupational Therapist. This role allows me to study at Sheffield Hallam to complete my OT degree whilst working.
