Currently working in intermediate care in the community in the High Weald, Lewes and Havens area (East Sussex), covering reablement, generic and neuro rehabilitation, management of long term conditions, palliative care, supporting hospital discharges and hospital admission avoidance (working closely with acute and community colleagues and MDT within the NHS and social care). Experience working in acute hospitals, and Brighton based community teams (including the specialist respiratory service) between 2008 and 2015.
