I am a highly organised, dedicated and passionate rehabilitation assistant within the NHS. I come into contact with different patients on a daily basis many of which have dementia and liaise with family members and outside agencies to ensure the discharge pathway is a smooth process for the patient. A key strength of mine is communication skills, which I feel will be extremely valuable to any role. I can work within a team or on a sole basis to ensure I complete the tasks I am challenged with. My job role consists of visiting patients on a regular basis from a occupational and physio therapy point of view providing sessions to help the patients gain independence back with their mobility. I appreciate how much having independence means to my patients so helping them on the pathway to what they were before they came into hospital in terms of mobility, daily living and functioning is of a main priority for me. I regularly provide activities like breakfast club for the patients to give them some form of normality supporting them to make their own breakfast and hot drinks I’m very passionate about providing a service to improve the people I’m caring for ways of life. I regularly use systems to access personal information in patients records to help with my sessions so understand the importance of GDPR and confidentiality.
