• HCPC registered Occupational Therapist with experience of working in the community with adults with physical and mental health impairments. • Experienced in conducting comprehensive strength-based OT assessments within the community under the care act 2014. • Experienced in planning and implementing a range of OT interventions including intensive rehab, provision of a broad range of equipment, minor and major adaptation, providing information and advice, recommendation for re-housing as well as care and support planning. • Experienced in working within a multi-disciplinary team to safeguard clients and also to facilitate the achievement of patient outcomes.
