Experienced Occupational Therapist with a demonstrated history of working in private, hospital & health care sectors. Working with adults and older people who have complex care needs, requiring comprehensive assessments, care management, and moving and handling advice and coaching. Identifying solutions for people who have short term difficulties, goal setting and re-ablement. Supporting people to find solutions within the home using equipment, minor and major adaptations, identifying and supporting people to obtain Disabled Facilities Grants. Working with people and their family and carers with acquired injuries and chronic conditions, facilitating independence where possible.
