I have worked as an NHS pharmacist since I graduated university in 2004. Currently I hold the position of Lead Pharmacist for Unlicensed Specials and Medicines Value, and Paediatric Formulary Interface, funded in part by South East London ICS, based at Guy’s and St Thomas’ NHS Foundation Trust. In this role I lead on developing a strategy, workplan and pathways to optimise and harmonise the prescribing of unlicensed specials to both paediatrics and adults across the ICS. Using a bespoke prescribing dashboard, highlighting primary care unlicensed specials prescribing data, I identify and target actions at both the highest spending medicines and those with the highest variety of formulations, across paediatrics and adults. I have collaborated and engaged with clinicians across all six boroughs of SEL, including senior medicines optimisation, PCN and practice-based pharmacists, as well as GPs and prescribing leads. I have developed resources including prescribing pathways, optimise RX messages and educational bulletins, to support and guide prescribing across the interface. I provide specialist advice to the management of medicines of complex children within the area, whilst upskilling colleagues in primary care to have the confidence and knowledge to do so in the future. I showcased my work within SEL ICS at the Royal Pharmaceutical Conference in 2023, and at the Neonatal and Paediatric Pharmacist Group (NPPG) conference in 2022. This followed on from November 2021 where I had two pieces of work accepted for presentation, the first titled ‘A project to rationalise the prescribing of unlicensed specials for children in a UK Clinical Commissioning Group’ and the second ‘NPPG & RCPCH position statement: Using Standardised Concentrations of Unlicensed Liquid Medicines in Children. How well has it been adopted nationally?’ I am now part of a small working group within NPPG seeking to extend the nationally recommended list of liquid concentrations, in both paediatrics and adults, something which is closely linked to, and will benefit my local role within SEL ICS.
