If you are a new patient, please arrive 15 minutes prior to your scheduled appointment time, and remember to bring your photo ID, insurance cards, and referral, if your insurance requires it. We will collect your copay or any balance due at the time of your appointment. To avoid a $50 fee, please cancel your appointment 24 hours prior. Ruby is a Board-Certified Family Nurse Practitioner who graduated from Olivet Nazarene University Nurse Practitioner program. She completed her undergraduate degree at Purdue University Calumet. Ruby’s first language was spanish. As a Registered Nurse she spent many hours translating for Physicians she worked with, this made her realize the need for spanish speaking providers and encouraged her to become a Nurse Practitioner. Ruby is passionate about patient education and providing compassionate care and has an interest in treating acne and prevention/early detection of skin cancer.
