Currently working as a Clinical Prescribing Pharmacist for WBC PCN. This role involves providing medicines optimisation services, ensuring the quality of care and patient safety, in line with local and national guidelines. This role enables one to manage people with long term conditions, advice on polypharmacy and medication adherence. I am best placed to provide healthcare information to clinicians, based on evidence-based medicine and also play an important role in integrating primary and secondary care. Currently, undertaking an MSC programme to advance as an ACP. Though a generalist, my interests involve managing LTC with an emphasis on cardiovascular health and CVD prevention. Other interests include care home patient management and medical education. I work as a Heart Failure Clinical Champion for West Midlands Academic Health Service Network, to support the deliveryof NHSE commissioned programmes, particularly CPIP and Heart Failure @Home. This role would involve supporting the LTP implementation framework to engage with patients with heart failure and heart valve disease to be supported by multidisciplinary teams as part of primary care networks. My previous patient-facing role was as a Community Pharmacy Manager with over 10+ years of experience in managing retail teams, as well as, their training and learning developmental needs. I managed a business with over a million pounds with YOY growth of over 20%. As part of fulfilling the NHS England pharmacy contract, I was involved in providing services such as travel clinics, flu clinics, health checks, substance misuse clinics, medicines use reviews and new medicines services. I have worked in line with the Department of Health target groups as people with respiratory, cardiovascular, people on NSAIDs and NOACs (Novel Anti-Coagulants).
