• Provide leadership to the NHS GM Practice-Based Medicines Optimisation team (Manchester) • Provide leadership across assigned PCNs to support Medicines Optimisation development and delivery • Development of the medicines optimisation role within primary care e.g. the potential to introduce consultant pharmacists in primary care • Deputise for the Deputy Head of Medicines Optimisation (Operational) • To support the development of Primary Care Networks (PCNs) and management of the medicines optimisation service. Support the delivery of the Medicines Optimisation work plan • Support the delivery of practice-based medicines optimisation key performance indicators • Support development and ensure implementation of the medicines optimisation strategy and operational plan • To work collaboratively with the NHS GM strategic medicine optimisation team (Manchester). • Support development and lead implementation of local and national policy • Support the development of the practice-based medicines optimisation team to meet the changing needs of the city • Support the management of the financial and clinical risk associated with medicines through the promotion of cost-effective and evidence-based prescribing • Ensure the implementation and monitoring of medicines optimisation targets and initiatives including QOF, QIPP and the locality and GM targets, and PCN work plan • Develop an effective working relationship with all healthcare professionals within the primary, secondary and social care teams so that knowledge and expertise is shared and consistency achieved across health and social care • Support the development, implementation and monitoring of guidelines, procedures, protocols and standards to ensure safe and cost-effective use of medicines in line with local priorities, national directives, medicines legislation and professional ethics • City-wide medicines optimisation lead for specified areas • Provide NHS GM representation to Manchester and Greater Manchester groups and committees • Provide expert advice on pharmaceutical matters including medicines related queries, cost-effective prescribing, medicines legislation, professional ethics and Freedom of Information requests • Responsible for medicines governance, risk management and ensuring compliance with medicines legislation. Respond to incidents related to prescribing and the supply of medicines • Provide professional leadership and mentorship to assigned members of the practice-based medicines optimisation team
